Aged Care Access Initiative
Program Overview and Rationale
In the 2008-09 Federal Budget, the Government announced that funding under the Aged Care GP Panels Initiative would be directed from July 2008 to support primary care service provision in residential aged care facilities through the Aged Care Access Initiative.
The Initiative aims to provide services from Allied Health professionals for low care residents permanently residing in Residential Aged Care Facilities (RACFs).
Duration of Program and how it operates
The Division received funding under this Initiative from GP NSW until June 2010. Based on a needs analysis with RACFs, psychological services were offered in July 2008 which was similar to the process of referring patients under the BOMH program. In August 2009, the Division extended the scope of services to include Physiotherapists and Exercise Physiologists, and it was determined that both GPs and RACF nurses could identify suitable residents to refer to contracted providers.
Physiotherapists
· Each resident referred will be entitled to 4 x 30 minutes physiotherapy sessions.
· Equitable access will be managed on a prorata basis by the facilities.
· GPs or Residential Aged Care Facility (RACFs) Registered Nurses (RN’s) can identify a resident who requires physiotherapy treatment.
· Residents require a referral from their GP prior to participating.
Exercise Physiologists
· Each group will run for six x 60 minute sessions.
· Residents require a medical clearance from their GP prior to participating.
· GPs or RACF RN’s can identify residents who can participate.
Psychological Services
· Each resident will be entitled to 4 x 60 minutes psychological sessions.
· GPs or Residential Aged Care Facility (RACFs) Registered Nurses (RN’s) can identify a resident who requires psychological treatment.
· Residents require a referral from their GP prior to participating.
How the Program benefits GPs and their patients
This program provides referral options for GP and RACF residents. It also assists low care Residential Aged Care Facility residents to access services that they might not otherwise have the ability to arrange or be able to afford.
Current level of Program participation / utilisation
All Central Coast GPs and low care Residential Aged Care Facilities are able to access patient referrals for low care residents via the Aged Care Access Initiative program.
Please Note: This funding cannot be used to pay the gap on private health insurance for any allied health services or to duplicate State or Commonwealth funded services.
