Chronic Disease Management (CDM)
Each Practice Support Officer is responsible for the timely dissemination of information regarding all aspects of CDM, PIP and SIP Item Numbers, provision of workshops, templates, and resource materials to assist in the correct implementation and application of these items.
Practice visits are conducted on a regular basis, to assist GPs and Practice Nurses in efficiently conducting Health Assessments, and in completing the correct documentation of GPMPs, TCAs and their reviews.
The PSO also acts as a resource for other projects, assisting in the incorporation of CDM Item Numbers into program systems.
