APAC/GP Shared Care

Program Overview and Rationale

APAC/GP Shared Care refers to an arrangement between a General Practitioner and APAC to “share’’ the care of a patient with an acute/post acute condition in the patients usual place of residence.

The rationale of the program is to enhance patient care options and open new avenues for which General Practice can access patient care through Acute Post Acute Care (APAC) Northern Sydney Central Coast Health Services(NSCCH).

APAC Service (NSCCHS – Central Coast Sector)

APAC is a highly skilled & experience multi-disciplinary service that is underpinned by the primary care principles. It is a 7-day a week service operational from Gosford and Wyong. APAC NSCCHS has treated over 12,000 patients.

Duration of Program and how it Operates

The Program will be ongoing for the GPs and the residents of the Central Coast.

GPs who are registered under the APAC/GP Shared Care Program will be able to access the service through a single point of contact number between the hours of 7am – 11pm.

Key elements of the Program include:

  • Improve communication links between GPs, NSCCH and Central Coast community services
  • Improve GPs knowledge and understanding about APAC services
  • Ensure GPs registered with Program have access to updated evidence based clinical guidelines and consumables
  • Provide clinical support for GPs
  • Ensure Clinical Governance

Current level of Program Participation / Utilisation

The Shared Care Project was launched on 7th August 2006. As of the end of November 2008, there were 110 GPs registered to the Program with 214 patients referred. The most prominent diagnosis referred has been cellulitis, followed by pneumonia and infective exacrebation of COPD.
